Frontenac Lennox & Addington Ontario Health Team
KCHC was actively involved in the development and submission of the application for the Frontenac, Lennox & Addington Ontario Health Team (FLA-OHT). The application was submitted to the Ministry of Health in September 2020 and approval for the advancement and implementation of our regional OHT was received in November. The application approval supports KCHC and numerous health system partners and community members to continue to co-design a regional healthcare system focused on connecting and integrating care to keep people healthy in their homes and communities. The FLA-OHT seeks to be a proactive and prevention-focused entity that grounds its work in equity and collaborative decision-making. The FLA-OHT will be able to make the greatest impact on service delivery and health of our community by basing its work on equity and collaboration.

- Over 60 health system organizations and 100 primary-care physicians signed on to partner in FLA-OHT development and implementation
- Highlight 3
- The Transitional Leadership Collaborative, the FLA-OHT’s oversight group, was established; KCHC holds a seat at this table

- KCHC established as the FLA-OHT fund holder

- 7 KCHC staff joined various FLA-OHT Working Groups and Support Structures to help guide and support the implementation of projects
